Hi, Michael - I'm sorry to hear you are having issues. What options did you chose in the injector? Do you mind sending your form to Support AT Qdabra DOT com so I can take a look at it?
Thanks!

Right click the radio button and check the properties. You can set them to different values.

Hi - here's a blog post that might help: http://blogs.msdn.com/b/infopath/archive/2010/06/03/how-to-pass-querystring-data-into-an-infopath-form.aspx
I started watching the video, since it has been a while since I recorded it - but it looks like I demo'd a few different approaches. If you have specific questions, let me know and I'll ...

I've not seen this happen before. I use IP 2013 if you want to attach the template to a reply (under the options tab) and I can see if I can open it. May help you figure out if it is the form or the specific Office 2013 install. Can the user open the form in design mode and try previewing it?

Yeah, people pickers are a bit of a bear that way. I think I'd probably add a field to the data source (doesn't have to be on the form canvas) that is invalid if the conditions are met. I'd add a little calculated value next to the people picker that is a red asterisk, and hide it if the conditions are met. Or, you could use a ...

So - you are just needing to display the data some place?
We have query fields in SharePoint list or library connections. A little kludgy, but what if you added 3 connections to the list or library. One for querying records where Originator = Current User. One for querying records where Supervisor = Current User. One for .... you get the ...

One common option is to set the form up to also submit to the library it is published to. You can promote fields as columns in the list. When the user submits the XML created by the form, the columns are populated with data from that XML. You can use a formula or form field for the form name to help ensure uniqueness, so that submitted XML ...

It is true, you can't join data sets in SSRS - there is a Lookup function in 2008 R2 and up - http://technet.microsoft.com/en-us/library/ee210531.aspx
