I guess my first question would be - is your calculated column displaying the value you anticipate?

As long as the user has access to the site. You could publish it as a content type, too, if you don't want a form library created. Are you intending this be used a browser or a filler form?
-
So - are you submitting the Employee ID to the Exceptions table from your form? Or are you maybe submitting the name?
-
I'm not sure if this can be done OOB - have you seen this?

The problem is that InfoPath doesn't offer any way out of the box to add rows to a repeating table. You either need to use code (or qRules) - if you don't need to save the data, you could just display the relevant rows from your secondary data source?

Hi - the issue is that the submitted data won't include anything from the secondary data sources. It would have to be copied into the main data source some how. Then, for repeating data, you'd have to promote each field as "merged". Given that, look around for "double eval" formulas - you could add some main data source ...

I wouldn't expect it to work when saved as a file - try publishing it to a library and then providing the users with a link to the library the form is published to.

It definitely needs to be published at least as Domain trust. Your users are opening the form from the published location? I ask because I see that it is published to a personal SharePoint site. Is the SharePoint server in their local intranet? From your post, it sounds like some users can open the form and some cannot, is that correct?

So, the attached sample has a rule on the qRules finishedLoading node and on the repeating field the multi-select is bound to. I've also added a helper attribute to the main data source that I would usually put in a form logic XML. The repeating field is set in the defaults to not be included by default.
