qRules can do this easily (free trial here: http://www.qdabra.com/en/products/download/qRulesTrial.aspx), or you can write custom code (here's a sample: http://sharepointbeeeye.com/infopath-2010-a-better-file-attachment-solution/)

If you find that the issue is mainly on machines with newer versions of IE, try putting the site into the compatibility view settings and see if that makes a difference.

Okay - so it is a SharePoint list form and you are correct, there are no rules that should be impacting this field - but it is a rich text field - and I have recently seen some issues with these in IE 11 (but only in some very specific circumstances) - what version of Internet Explorer are your users on? Is there any correlation between IE ...

And to Save, you just use the Ribbon at the top of the form? Any chance I could see a copy of the form? You can attach it under the Options tab in a reply.
-
Do you fill out the form in a web browser? Or does the form open in InfoPath on user's machines?
-
Is this a browser or a filler form? Are there any rules that may affect the field? You can right click the field in the data source task pane and select the Rule Inspector to see.

You need to add multiple instances of the Item group, then in each instance, set the individual Utensil fields. I'm sorry, I don't know how else to explain it - does looking at the defaults in the sample I sent earlier help at all?
-
Just click the plus by each repeating item in the default values, then click on each field you want to enter a default for and type in the default. The "OK" button will close the dialog, so don't click it till you are done. :)

Pop your form over to me via email again and I'll take a look at your logic, okay?
