So, there is a new item, with (no title) or some such? You are displaying the columns you've mapped in the view of the list you are looking at? Can you reply and attach a screenshot of your ShP list (under the Options tab, you can attach a file)?

Sorry, I should have been more clear. Yes, when the form is submitted, a new list item is created in the sharepoint list, but no values are populated.

Hi -- when you say "none of the fields is properly filled out" but a new item is created, what do you mean? Do you mean there are unexpected values in particular fields? No values?

You are correct.  I don't think the users understand that they don't really NEED to do this at all.  They will need to connect to our network ANYWAY to get their email, etc. I am going to make them understand this. Thanks for the response!
