I was able to get it installed.  For some reason I had to select "Just Me" instead of "Everyone" for who has access to application.  Weird.

I published my Infopath expense report form template to a SharePoint form library.  I noticed that when I filled out a form the value that appears in the Name field in the SharePoint form library is the document id.  Is there a way to change this to be the employee name instead?
