Insert is tricky - Your repeating node has to be the last item in the parent group. In your form schema, move grpMarketCountry below grpMarket.

I think I may be confused by the word "updated" here - I keep thinking you are talking about updating existing list items from the form. Is that the case? I think you said that if you fill out a new form, new items are added to the list correctly?

The mapping file will need the site URL updated if that is now different, yes. If the column names have changed, the mapping would also need to be updated.
If an XML has values in the field mapped to the SharePoint list ID, qRules is going to try to update the existing item - so if your form has, for example, an item with an ID of ...

Okay - can you verify that the item with the id number of 420 is still in the list? Your upgrade didn't change any ID numbers, did it?

That said, I'd guess that the issue is going to stem from your lookup columns. Is the text in ee_Department a value that is available when you manually add ...

Save one of your already filled out forms down from SharePoint. Then, with your form open in design mode, go to File > Advanced Form Options and look for the preview settings. In the center of that dialog, you can browse for a file to preview with - and then preview your form - it will ...

Hi - Your screenshots aren't showing. But I would think you would to use a default value for ManagementApproachWriters with your double eval instead of having it on a button click? Another option would be to change the form Submit to use rules, and have your first rule set the field, just like you do now with your button click.
