Under the Data tab in the ribbon, find Default Values in the Form Data section. If you drill down to your repeating group, you'll find options for including more rows - see attached screenshot.

So you were submitting multiple values - in which case, yes, you need the double eval formula. I don't know about the newer versions of U2U, but the one I attached has the UpdateListItems method - you can test your batch and it can help figure out what you need to submit. That's usually how I figure out some of the trickier edge case ...

I've attached a copy of your form with the filters on the dropdowns modified. Since ChooseOptionalPermissionsFields is your repeating group, and the context for the dropdown is that repeating group, the filter looks like:
../d:Title = current()/my:OptionalRole
Save the form locally. Right click and select Design to open in design ...

I think the issue here is that "current()" is not being used in the filter.
Where is the dropdown located? Can you attach your form to a reply under the options tab?
Generally, I'd guess the filter should be:
../d:Title = current()/my:ChooseOptionalPermissionsFields/my:OptionalRole
But that is just a guess.

In the filter on the dropdown, change the first dropdown to "the expression" and copy the XPath out of the text field that appears and paste it into a reply, please.

When you create the document are you adding the InfoPath processing-instructions so the XML knows what template to use? Maybe attach one of the XML you are creating to a reply?

Your first concat statement should be correct - unless you are trying to submit multiple values, I'm not sure why you'd use a double eval?
I'm attaching a zip with the free U2U CAML query builder - it is an older version that has the UpdateListItems method. Test your CAML to make sure you have the syntax ...

This will require code - so either the Sandbox Service must be running (and you must be a site collection admin) to publish directly to a form library, or you will need to admin approve the form.
http://www.bizsupportonline.net/infopath2007/4-way-programmatically-add-row-repeating-table.htm has some code samples.

Some column types are trickier than others. Lookups also can use the -1;# syntax to look up the ID of the item - so, if you had a lookup column that displayed the title and you wanted to submit to it, but didn't know the ID, you could use -1;#WhateverAValidTitleValueIs

Awww. You made my day - hadn't been called "magic" before. :)
