take the document list template, and resurrect it onto a higher level to see if that makes some difference, i dont think that should be of any concern

relinking such documents back to library is a better solution, to relink the documents to the document library open the document library -> settings -> document library settings -> relink documents to this library -> and select the documents to be relinked. to identifying which forms are good are which are bad, for the good ...

why dont you just enable the save options, to do so open your form template goto -> tools -> form options -> open and save and check the Save and Save as check box, publish the form now save option should be available to the user

try signing the form template, to sign the form template goto tools-> form options -> security and trust.

multiple users can open(for read/write) the same form in browser with out getting any message. and if userA and userB open the form, then make changes and userA saves, then userB tries to save, he will get a message that the form has changed... but i wont recommend using this.
