Yup, it makes sense - and yeah, you can only add one attachment to a post, so multiple posts is the only choice. :)
Use rules for submit. Add a rule that emails User A. For conditions, I think I'd just make sure all the "signature" fields are populated. You can select "all instances" for a repeating field ...

There is, but it can be really complex. Here is a blog post that may help you: https://blogs.msdn.microsoft.com/infopath/2006/11/27/conditional-default-values/

Well - how many corresponding fields are there? Maybe you could use rules instead, set rules on the fields that would set the value, with the first rule to clear the field? Then you could have conditions as to when to set the field.

Your rule will "set a field's value", and the value will be the division or region (this will only affect the current repeating node, the one being inserted) and then use a formula to get the value of ...

I have not seen this before. Have you tried deactivating / reactivating the template? What about if you set up a new (test) library and add the content type to the new library? What happens then?

So - the point is to only show one of those at any given time - they are all bound to the same field. You just need to add to the conditional formatting so only the one with the value options you want users to be able to select from is displayed. Does that make sense?
