The form data might be sent as attachment. go to tools-> data connections modify your submit connection

the code should work perfectly i guess your form falls under the scenarios for which this code doesnt work as mentioned by jimmy in the blog. if it doesnt please attach a skelton of your template (with code) so that we can have a look at it
-
check if there is some code in
FormEvents_Save(object sender, SaveEventArgs e)
this gets fired when form is saved
-
relinking the old forms should do the trick, go to the document library -> settings -> document library settings -> relink documents to this library
and select the documents to be relinked.
-
for such cases i use (c# code)
string abc =Microsoft.VisualBasic.Interaction.MsgBox("message",Microsoft.VisualBasic.MsgBoxStyle.YesNo,"title" ).ToString();
if(abc.equals("Yes"))
{
//yes was pressed
}
else
{
//no was pressed
}

you will need to first deactivate the form template from the site in central administration. then upload the new template. then activate it again.

there is difference between saving a form and submitting a form,
when we say saving it means Work In Progress copy. so it can contain validation errors.
but when we say submit it means completed copy, it should not contain validation errors else it wont get submitted.
that is why we cannot directly use a submit connection to save data

if you are using page breaks, then this is not possible. if not then simply in the properties of the sections set the size-> height to auto or 0. that way if the control is hidden it wont occupy space on the page
