I have a SP List of Library periodicals that gets routed. I need to create a Request form for these materials and not a form to add an new Item to the List.
Is this even possible? I'm not sure how to implement such a beast.
Thanks for any suggestion,
John

We canceled this topic because importing the form history (database) is difficult and complex.
What we want to do now only is to move the forms we used in sharepoint 2010 to sharepoint 2016.
Is it possible to can you tell me step by step how to transfer the forms we use in 2010 sharepoint to 2016 sharepoint?

I made a guess on which dropdowns were involved and added calculated values that I think will work for these. Save the attached file locally. Right click, select design. Then preview to see if this works the way you need - if not, let me know and we'll try again. :)

Oh, it is a list form, which slightly changes the answer, since you aren't dealing with repeating data. Is it the Credential Status that will have the options of "Approved" etc?
