Hi,
I think you didn’t give “http:” before site address while taking submit data connection.
Site address should be: “http://Sitename/subsitename/Document folder name”

Right click on the table and select “Table properties”.
Select Row Tab.
Select “Automatically set row height” radio button and click on ok.

If you want to insert same company name for multiple forms then use following formula for file name.
