You can access submit options from the tools menu. This is what the toolbar button uses.
Then you can switch to submit using rules.

So are you getting the ID returned? but the filename not reflecting it?
I would try making it visible in thebrowser form to test.
I would try adding a field to your form and use that for the filename to test also.

Welcome to the forum.
Typically the most common issue is that the GUID for the list is not correct for the path. Make sure you are referencing the specific site path for the list.
There is an update and delete option for the CAML in addition to insert. I think there should be some examples

Sure.
Add another submit data connection for email.
Then you just need a field for the email address or pull them from your form or data.
You can add a rule to a button to submit the email DC and/or reset the form.

You can have a header in IP 2007 but I believe it would use the first value of a repeating group and not itterate through each page.
You my need to put your own control, repeating table, on your view and use page breaks to replicate what you want.

You should be able to query a secondary data connection to your table and get the max() id to get the latest record submitted.
Then yes, you can move that unique ID into your form and use it in the filename.

Do some research on the current() function to reference the dropdown on the current row.

If you are using managed code typically you just start in visual studio and then use the tabs to move back and forth between the code and the form.

Not sure exactly how you are architecting this but I will give you some feedback.
1 - I would analyze whether you need a second form. Typically if the data is the same I would add a second view to the form rather than trying to create a second form and all of that complexity.
2 - In these scenarios it works best if you have a common data ...

Hey Patrick.
You would just need to backtrack.
If you know the values then just use a filter to get the display field.
concat(txt1, "x", display[value=ddl1], "@", display[value=ddl2])
So you just need to reference your secondary SP lists and look up the display field value where the value fields = what was selected.
Hope ...
