Sometimes when I'm having trouble with conditional formatting, instead of setting the rule to hide the control, I set it to change the fill color. You can use different colors for different rules. Maybe that will help you see what is going wrong.

For item a - are you connecting to a SharePoint list? I'm not sure what an "SP drive" is. If this is a SharePoint list, I'd set the query field to the value of the text box, then run the query, so you get back just the item you need. You don't want to be querying 48k items from a form.

So - I went to create a sample form (attached) and I did notice that since my conditions are "OR" conditions, after saving & re-opening, IP created multiple rules (one for each condition) - but I don't think that's what you are talking about here... at any rate, you can see that whether the section is displayed depends on ...
-
I keep thinking the issue is that with the translate, only the first item is being compared... It looks like you are comparing items from two different lists, is that correct? Can you just add a calculated column to each list with the lower case value of the field?

Generally when I have trouble with conditional formatting it is either because I have my and / or mixed up, or because the fields don't have the values I think they do. Are Field1 & Field2 Boolean?

Need more info, I'm afraid. When do you get the error? On submit? Or when you try to open?
-
I think your rules are in the wrong place. You have rules on the GCDO trial type field - I think those should go on the Submit Rules under the data tab in the ribbon.
-
I need more info on the problem. The form is set to submit when the GCDO trial type is selected - is that what you want? for the form to be emailed at that point? What is happening? Is it not emailing?

I'll try to take a look sometime today. In the future, could you start new threads instead of adding on to old threads? Thanks!

The data is in an XML file. You can go to Data on the ribbon, select Resource files, and then export, edit and re-add the file.
