qRules hasn't been tested with browser forms.

I can't provide advice on the code you'll need to write, but I can suggest a codeless solution. (If your form is codeless) You can inject qRules v1.7 into it and get CopyTable functionality to cover this.

Use that function to set the value of a field only when the form is first created. Then use that field as your form name.

I recommend using expression boxes to show your text. It comes across much cleaner in email.

The function you are using as a form name will always create a unique (new) value. InfoPath will overwrite, but only if the form name already exists. You'll need to modify your submit in order to use the same form name in future edits.

The solution given to you above works in InfoPath 2007.

When creating an email submit connection you can tell it to include the form as an attachment. Please clarify on the second requirement. Is the radio button read only, do you expect the user to be able to use it?
