Yup, I meant the XSN. I extracted the source files, searched for ReferenceUBADepartments and can't find it either. Do you fill this form out in the web browser or in the Filler (InfoPath application)?

If so, and if this form is trying to query SharePoint web services, that would be the issue - those are blocked on SharePoint Online. If you are using SharePoint on-prem, check the SharePoint ULS logs for the correlation ID (or provide it along with the date/time of the error) to ...

 So - the basic syntax is: concat("Here is my text: ", field picked from field picker, " and here is more text ", another field picked from field picker)  Does that help? :)

All 3 views are identical except one is read only and they have slight differences in the signature boxes. I opened the "upgrade" file and that MAY have something referenced, but I have no idea what that is? It's not a view. Any guidance?

This must be a list form - right? Can you tell me what is not working in your concat, or attach your form under the Options tab in a reply, so I can take a look?
