The intention is to use the Web Browser. SharePoint Online doesnt agree sometimes and opens InfoPath, but we don't want that. When I'm trying to create a new form in SharePoint, it is opening up the application and then giving me those errors.

Yup, I meant the XSN. I extracted the source files, searched for ReferenceUBADepartments and can't find it either. Do you fill this form out in the web browser or in the Filler (InfoPath application)?

Hi,
I use InfoPath 2013. I have 2 multi select fields.
Practices multi select field has values: A, B, C
Offerings multi select field has values: X, Y, Z - NOTE: This multi select field is hidden until C is chosen in Practices field.
MY REQUIREMENT:
If "Practices" = C, then "Offerings" show up and I select values ...

Hi - sorry I missed this last week. Are you using SharePoint online? If so, and if this form is trying to query SharePoint web services, that would be the issue - those are blocked on SharePoint Online. If you are using SharePoint on-prem, check the SharePoint ULS logs for the correlation ID (or provide it along with the date/time of the error) to ...

All 3 views are identical except one is read only and they have slight differences in the signature boxes. I opened the "upgrade" file and that MAY have something referenced, but I have no idea what that is? It's not a view. Any guidance?

This must be a list form - right? Can you tell me what is not working in your concat, or attach your form under the Options tab in a reply, so I can take a look?
