In the data source taskpane, add a new field anywhere in the data source and call it submitted. Specify its data type as True/False (boolean) and leave the default value as false.
Right click the button you want to use and open its properties. Click the Rules... button.
Click Add...

You can place a boolean flag field linkGenerated in your form, and a button that when clicked, performs the concatenation you mentioned and sets linkGenerated to true. Then use conditional formatting to hide the hyperlink when linkGenerated is false, and hide the textbox when it's true.

I know of no way to accomplish this in the InfoPath designer, but for a fixed number (in your case 3) this is probably the easiest way to accomplish that:
Save the template as its source files (from the File menu in the InfoPath designer)
Open the schema (XSD) file in a text editor
Find the line that refers to your repeating field ...

I have no idea what you mean when you say "extra brackets." To what are you referring?
If you don't want the NaN to appear, you can add conditional formatting to the expression box to hide it when @cust_dob is blank.

Could you give a little more description about what kind of situation you have where you want users to be able to insert hyperlinks?

Great to hear that!
It looks like your main data source is based on a database query connection, rather than being built from scratch, so in such cases, the myFields node uses the dfs namespace. That's something I didn't really know either, so it's a good thing for us both to keep in mind :-)

If it's not too much trouble to share it, could you attach a copy of the template.xml file from your extracted template source (use the Options tab when you reply)? That will allow me to see how your main data source is structured and figure out why it's not finding that field1.

I see. I guess you are using InfoPath 2003. Please back up your XSN file and try the following steps to remove the code from your form:
1. Save the template as its source files (from the File menu of the designer), close InfoPath and browse to the folder where you saved the files.
2. Open manifext.xsf in any text editor.
3. Delete ...

Just copy the part in single quotes starting after the comma and going up to the end parenthesis. In my form, the full line looks like this:
var Namespaces = 'xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" xmlns:d="http://schemas.microsoft.com/office/infopath/2003/ado/dataFields" ...
