When you promoted those fields, did you let the wizard create new columns for you, or did you match them up with existing columns in your form library? When you try to edit your document, your metadata is present?  

 Hi, I don't think there is a setting for this. If you want to set the height you can add a table and set the minimum height on the row properties.

 Can you attach your form, when you reply, click on option tab, ...

I think you have to check your conditionnal formatting, and test it on preview, if can can't debug it, please attach your form template.

 As I told you before with the rules you will be able to modify the text only not the inner xml, so if you try to add "<img ..."with rules, he just add  <img...to display it The inner xml can only be modified  with codes.

 Yes that's true, but I just see a solution with code, if you want to do that "dynamically".

I,  check the value of your drop down on your Template form, sometime, the display value is bind with a title and the value with the ID. The value is publish on Sharepoint.

 Hi, you can add another submit button that do what you want and display it with conditionnal formatting when your first approval is finish. When your user click on the first submit you can set a field to true. You can hide or show your button base on this field with conditionnal formatting.

 Hi, the form is just an example, I use a xml as secondary data connection, but it can be a sharepoint list. For the drop down, you can bind it to your sharepoint list and then you can show all your supplier.
