Hi Patrick, Thank you for your email. I have added the data connecton to the sharepoint library. And I have put a send to webpart comand on the button, However this does not seem to work. Any other ideas would really be appreciated. Thank you

Hi Patrick,   Thank you for your response. Please find attached my form. Within Part C, there are 3 areas in which an email address could be added however only one option should be applied depending on which option is selected. Kind Regards Kully
