Hi,
Follow the below steps.
1. Open the form in design mode.
2. Go to Design Tasks pane.
3. Click on Controls.
4. At the end of the pane you will see a check box with name ‘Automatically create data source'. Check this check box.
5. Now drag and drop the controls. It will automatically creates the data source for the controls.

Hi,
Add the condition as if CommentSubmitted = TRUE and commentsField is not blank then make the field read only. Also make sure that the CommentsSubmitted field should be in repeating column in the same level as comments field.

Use the following expression in the data validation of Employee Id field.
. = ../preceding-sibling::my:group2/my:field1 and . != ""
Here my:group2 is the repeating node and my:field1 is the employee id node.

If you want to customize the options in the tool bar, you need to check/uncheck the check boxes available in the form options -> browser tab as anuma said. It contains Submit/ Save/ Save As/ Close/Views/ Print View/Update. You can show / hide these buttons on the tool bar.
