I'm not sure if this could cause issues, but maybe you don't need to launch Infopath from the template and thus making a new form.
Instead, save out some already opened and saved forms from the template, but fill in the html path in one of the fields different for each one.
Then when the user uses your link in the flow chart there ...

Is it on a particular user's computer?
I've never had the buttons stop working, but I wonder - are you fiddling with form internals (extracting, recompacting etc). I could imagine that a button might get 'recreated' with a different internal ID, and then any code behind the button might no longer work as it's expecting to be ...

The way your doing it seems easy enough - you File -> Extract, go to extract folder, delete, then re-open the xsf file in the extracted foler in design mode and File -> Save/Publish and Infopath will recombine all the files into a single xsn.

Sorry, I don't use sharepoint, but to give you a reply in the absence of others so far:
Could: var strSiteURL = XDocument.DataAdapters["Announcements"].SiteUrl
give you the sharepoint url?
or
XDocument.UI.Alert("URI: " + XDocument.URI);

To set margins: right click section and select properties -> size tab -> put 0 px in all the margin boxes
To set borders: right click section and select borders and ...

Put another 'view' in your form - View menu -> Manage Views - from the panel on the right on your Infopath window, select 'Add a new view' from the bottom.
It's like another 'page' in your form.
Then, on your first/main page, create a button control - right click it and go to it's properties -> rules ...

Sorry, I don't understand your second question.

I gather the submit button isn't actually a 'send via email' button because then the user wouldn't accomplish anything by going back and changing fields, because it is already sent!
Maybe a 'preview' button is what you mean - which you can do by designing a view a bit like a 'print view' which consists of all the ...

My instructions weren't super clear - or perhaps, read them again -
Try saving the template with the same filename but on the publishing wizard screen where you change the directory and filename for output, change the FORM NAME box (it's below the filename box) to something else - old forms should still connect to the template ...
