Both will work. I think Submit to Sharepoint library is much simler to impliment.

U will need to create intermediate field for filtered information. Right before submit populate that field with expression similar to following:
substring-after(eval(eval(office, 'substring-after(concat(checked, ";", value), "yes")'), ".."), ";")

The LastName and FirstName are your hidden fields they are not part of the formula. And here are the expressions to set your hidden fields to the new values. You will need two separate actions for each of your field.
substring-before(DisplayName," ") - which is to get the last name...
substring-after(DisplayName," ") - to get ...
-
In most companies DisplayName is capitalized already. So your expression should be much simplier. For example
LastName: substring-before(DisplayName," ") FirstName: substring-after(DisplayName," ")

I think better design instead of button would be removing button completelly and assign set of rules to the field with numbers. Each state will require 1 rule with condition.

Sorry hadn't realized that maximum amount of replies could be any figure. Here is condition for disabling control after the limit of 10 replies is reached.
string-lenth(xdMath:Eval(xdMath:Eval(my:MyRepeatingSection, 'substring("|", 1, number(my:UserName = userName()))', ".."))>9
It's the same ...

I would create additional text field (lets say UserName) at the same level as your feedback field. Feedback field's rule is setting value of UserName field to the userName() function. In conditional formating properties of your repeating section i would make repeating section disable under following ...
