Do you get an error when you try to open the example form? What version of InfoPath are you using? Did you download the file, then right click the downloaded file and select Design?

What I am suggesting is that you look under the Form Options, and check Compatibility. Enter the server URL for verifying and go to Design Checker and hit refresh.
