Not sure about form templates in sharepoint, or even Infopath 2007, but I found the following avoided my users getting 'form upgrade' messages in Infopath 2003.
When you publish a form, you end up specifying a form name and a form URL to publish to. I always adjust the form name field so that it reads something like ...

Oh, and another thought.
Have two repeating tables for the same repeating group under each other and delete the columns from each table that you don't want.
When either table has a row added, the corresponding row will appear in the above too. But whilst this matches your described requirement, I think you will find it's not ...

Well, you can't have the 'header', but you can certainly put two text box controls one under the other in the same cell and they will hold the two separate values.
Your header could be something like Header1/Header2.
e.g.
Header1/Header2 Header3 ...

On repeating tables in the properties window you could use Default Values or 'rules when user adds/inserts a row' and set each field to the value in your field outside the table. No code needed.

Consider fields to store a toggle value when a button is pressed - I like using -1 and 1 so my button rule can just be current value of field * -1
Then put conditional formatting on the table rows (or a section within which the whole table is placed) to be hidden or not depending on the value of the associated toggle field.

I can't do this in C, but I can in jScript/vbScript...
So in case you can use this method. JScript.var strCustName;
var nodesToIterate = XDocument.DOM.selectNodes("//my:entries");
for(i=0;i<nodesToIterate.length;i++)
{
strCustName = ...

Hm. interesting proposal. Don't really know why you would need to do this though.
But I love a bit of fiddle for fiddles sake.
Working example attached! :-)
Gonna explain why you need this generic/dynamic query method?

Not sure if this is the cause, but when the first control on a view (like a button or textbox - and maybe even a scrolling region) on a page is part way down the view, Infopath scrolls to make it at the 'top' of the window.
Try putting a button, shrunk right down to a few pixels at the top left of every view, outside of the ...

I'm full of suggestions - problem is I don't always work out if it's the best solution. But try this.
Create a secondary datasource xml file with a structure like below.
Then for the expression box value, select the program field/node from
record repeating group/node in the myDatasource secondary connection
with a ...

<xdado> - is just the root node of the XML file, like <HTML> tag in a webpage.
Notice the closing node at the end </xdado> at the end of the file
<dfs:, <d: - ok, so XML files you may have seen might not have seen these letters/characters followed by colon before the node name.
These are called namespaces and ...
