Hi. I have never seen this occur either. I've had trouble where I cannot create relative hyperlinks in InfoPath because it always wants to prepend ''http://'' but I've never seen it add a third slash.
Will you please try to see if this repros in a brand new blank form? Also, if possible, see if it occurs on a different machine.

Something you could try would be to create a grid background image. That can act as a ruler by temporarily setting your view background image to that grid graphic.

Rules are run when a field value changes. Only the rules on the field that changes are run. Thus your rules should be placed on the field that you expect to change.
Access_field[@Access_record=current()] will only provide a value if Access_field itself contains a value... otherwise you will need something more like ...

Add a rule to the ''Departments'' drop-down list which sets the value of the ''V-Code'' list. That way the user doesn't need to select it themselves.
Create a print view and assiciate it with your edit view. The print view will display the cipher code name, while the edit view will continue to show the department name.
