These are different fields? You are using list forms? Just add a data connection to the same list, and set it to not query when the form opens. Sort the data by Modified desc. Then when the user enters a site, set the site query field to the requested site and execute the query. Use rules to set your current fields from the 2DS fields.

Missed that, sorry - but did you try "and"? Because what you are effectively saying with "or" is that if the data doesn't match this or that, it is invalid. And it can't match both. You could also consolidate the pattern. See the attached sample - save locally, right click and select design then you can preview. :)

I see no "Rec" or "Ship Loc" columns. Here are the columns I see in the form you attached.

Sorry - I'm not finding it on this template - where can I find it? Is it in the repeating table? If you can tell me where to look, I'd be happy to try to help. :)

Any chance I can see a copy of the form? Is the locations field using a dropdown or an external item picker or some other type of control?

You would need to add this tracking to the form yourself via rules, I think. You could have a rule on the option button that when it is clicked records the user name and the datetime the action took place, and display that on the form in calculated value or read only controls, if you wished. The data would be stored in the XML for the form so ...
