I believe you have a server issue of some kind with your sandbox code service - that the issue is not qRules, but with the sandbox service on your server. Any form with code published directly to a form library goes into the sandbox. If the processes are not starting / ...

Your screenshots cut off some of the text. I'd investigate why the sandbox service is failing to start, personally.

Is this in Office 365? Or an on-prem server? If Office 365, these web services can't be accessed from a browser form. If this is an on-prem server, there are settings that can be changed to allow access.

Follow-up:
The respondent to this record indicated the request could be withdrawn, so I opened the form (in the application), completed the notes stating it was no longer needed, and submitted it. Now it opens as a Web form - Yea!
Still, it would be nice to know why this happened if anyone has a quick explanation. Thx
