Typically this would be a permissions issue on the Database. Please verify the account you are running the install with and the Network Service or the account you are selecting for DBXL has administrative permissoins in SQL.
If this is a seperate Database Server try using a SQL account instead of a windows account.
You can email ...

OK, paying more attention now.
Try
Mail.Into = OSOA.text
Your assignments are to the fields not the values.
Also could add .text when you assign the variable so it it a text value.
Look at some other code example at how to reference field values in code.

I would insert a line for a MSGBOX(oOSOA) or Alert() etc after each line to verify you are getting the field values and the concatenated value is correct. Can't remember which command does a pop up message in VBS but at least then you can see what is transpiring. Is & the right choice for concatenation?

Welcome to the forum Scott.I am pretty sure, that with code, you could customize the Intro of the email to include your fields. mail.Intro but you should be able to search on the site here for posts on email and code.

In 2007 there is a combo box control. This can be populated by a data connection and the user can make selections. Otherwise you can have a dropdown and then build a list from subsequent selections too if needed.
What exactly is the situation and maybe we could be more helpful.

Typically we replace a button text with an image. Which I think Greg has a post in his blog. I am pretty sure this works in forms services as well.
You might want to check that out or give us more detail as to how you are setting this up.

Hi Dean.
Sorry you are getting an error.
Can you open Administrative tools and the Event viewer and Qdabra. Please paste in the whole text of all errors you are recieving.

This would be pretty tricky but probably possible via code.
Typically you would use a database or even save the data as XML from the first form and then have the second from pick it up and use it.
-
That is not really possible.
You can try opening XML in Word but it won't be too pretty.
They either need to get InfoPath, You need to use a Word template, Or you need Form Services to have the InfoPath forms browser based.
