Your syntax looks incorrect there. Remember, we need to wrap our field values in single quotes. Also, there was no space before your "and".
/where=Where StateID = '" , Field1, "' and CityID = '", Field2, "'")
So, that would be:
StateID = SingleQuoteDoubleQuote, Field1, ...

Wonderful - glad you have it sorted!

I'm a little OCD, so I just tried this. And my concat looks like:concat("ModifySqlQuery /dsname=GenStates /where=where StateId = '", field1, "'")
As you can see, I'm not using parenthesis around my where clause, and I don't have double quotes around my column name. This worked in my environment.
However, ...

I asked our tester if he recalled how he tested this - with a concat or not - he couldn't recall. We'll check it out and post back - it may be a day or so, okay?

Hi -
You probably want a drop down control for the first selection - if you give your users a text box, there is a good chance they'll type in something that isn't the exact same as the options you want them to have.
I've attached a simple sample - this is easier to describe with a form than in words. The template is IP 2007 - ...

Wonderful - mind posting your solution for others? Are you checking the InputParameters for the key? (e.InputParameters.ContainsKey)

What you are describing can be handled via input parameters. You will either need to write code or use an add-on like qRules to do this, however. Here's an InfoPath Team blog post on using input parameters.

Try this version - I opened the template you attached in design mode and removed the code - in IP 2007 & 2010, under the Form Options (from the Tools menu in IP 2003 & 2007) there is a programming category that has a Remove Code button.
Right click the link to the XSN in the header of this post, save it locally, then right click to open ...

When you publish your form to SharePoint, you should have the ability to select fields to promote. I know by IP 2007, under Tools > Form Options you could find a property promotion category, but can't recall if it was there in IP 2003

This command isn't very sophisticated - it is just grabbing whatever your current command is and slapping the passed in Where clause on it, so you know.
You seem to be missing a closing parenthesis, tho:
You've got an open parenthesis before "Forecaster", but no closing. Try this:
concat("ModifySqlQuery /dsname=Main ...
