I have a repeating table that lives inside a repeating section. I have a button that uses the qrules insert command to add a row to the repeating table.
However, when a second instance of the repeating section is added, when I click the "add" button, it inserts the row on the table inside the first section only.
Is there a ...

Patrick's comment just sank into my dense skull . . . I just realized you can email a HTML version of the form AND submit it to a library at the same time. I can't believe I didn't know this before. I'm still struggling with this. I would love to hear ANY suggestions to help me out.

Any other suggestions on how to troubleshoot this? I've bumped up my diagnostic logging for IPFS in central admin. But when I turn on anonymous access and verify the failure, no messages are kicked out to the event viewer (none regarding forms services, anyway).

It works now, sorry about the confusion. In the client, I am using a "dummy" user. In IPFS, my corporate user was able to submit the file fine, with the custom name. It's wierd, I can do the SubmitToSharePointList command with my dummy user.
Please understand that the anonymous access problem is not ...
