Here is basically what we want to do:

----------
Database1/TableA = retrieve data from here
Database2/TableB = write data to here

We need a dropdownbox with ItemNumber(db1/tblA) on form load. The user selects ItemNumber(db1/tblA) and some fields below this are populated with that ItemNumber's data from db1/tblA. This can be done right when the user selects a different item from the drop down, or after the user presses a select button next to the dropdown.

Now an item is selected and some fields below that are populated. The user can now enter a comment and select some other checkbox and radio button items.

Finally, the submit button is pressed and the data from all the fields is inserted into db2/tblB.
----------

I have tried everything that we can think of and everything we have found from researching and can not get it to work.

Can someone PLEASE explain how to do this, or create a sample form for me that does all of this.

Thank you.
