SharePoint Designer 2007 workflows

Env: SharePoint 2007
Using the Codeplex extension provided Paul Kotlyar called SPDActivities which you can download from http://spdactivities.codeplex.com/

Background:
I'm not a SharePoint expert but can help myself (18yrs apps dev). SharePoint was dumped on my desk a few months ago and I need to make sure it keeps working.
We have over 400 staff using multiple InfoPath forms. Most forms exclude custom code. Some of the forms need permissions to be locked down to only the "Create by", "Manager" and "HR" as it contains private information.

Issue:
The workflow that should delete permissions assign to the form is not running through "Time out". Could it be due to number of users we trying to delete from the item?

Workflow logic:
No Condition
Actions:
Inherit list permissions on "Current Item"
then Delete permissions assignment in "Current List" from "NT AUTHORITY\authenticated users"
then Grant "Contribute" Permission on "Current Item" to "Manager"
then Grant "Contribute" Permission on "Current Item" to "HR Group"

I've tried different Delete options in other word "Forms Contributor" all with the same result.

Sometime the workflow will run through but the permissions on the item stay unchanged and other times I am getting an time out error.
