Title column in SharePoint library with InfoPath template no longer populating

Hi all ---first time poster. I searched this board for an answer to this question and it doesn't appear that this has been encountered yet. Apologies if I overlooked it. I posted this question on MSDN forums as well and thought I'd expand my audinece here.

I've run into an issue with a SharePoint library that has me stumped. I have an InfoPath 2007 form published to a SharePoint 2010 library.

Normally, when the form is submitted, the title column in the library is autopopulated with the name you give the form at the time of submission suffixed with a '.xml'. This was working fine 2 weeks ago. I went on vacation and came back to resume the project and now the title column isn't populating.

I'm hoping there's something simple I'm overlooking...but I'm really stumped and was driven to posting a question here.

Add'l info:

The items that had the title column populated -- are still populated. Only new items are failing to populate the title column.
I have tried republishing the form with no luck.
I have published the form to a new library and no luck
I have another form on a different site on the same farm that works as expected
There are no event handlers on the form
There are no workflows on the library
