Certain library design on SharePoint

Hi All,

I'm wondering whether it is possible to create a crosstab in a document library on SharePoint like you'll find it attached.

On SharePoint I'd like to have a document library in which 10 documents are already stored.

Then I have a form

- with a dropdown-field in which the user can choose his ID (Member1, Member2, etc.)

- two columns (column A is containing 10 criteria; column B is for data input, in which the user can evaluate the corresponding criteria of column A)

-via 'submit'-button the evaluation should be send to the document library

-the evaluation of member1 should be shown in column 'Member1', the evaluation of member2 should be shown in column 'Member2'
