Blank screen when initiating a workflow with a moddified link - K2 - InfoPath form - SharePoint 2010

Hello there, I have a modified workflow initiation link for a K2 workflow hosted in SharePoint 2010. This worked for the last year and has stopped working for only a subset of people. When some users click on the modified link, they only get a blank page...no workflow submit view. When they click on the unmodified link, it works fine.

Default workflow initiation link, unmodified (The window they are left with after submitting the workflow is the Forms Library page)

Desired modified link (So the window they are left with open is a customized "You may now close this window" page rather than the Forms Library page)

Some patch or setting has globally been changed.

Does anyone have any insight to this situation? I've checked every combination of compatibility view settings, browser versions, OS versions and found no commonalities between the users that it doesn't work for.
