Create a form library and don't mess with it. Create an InfoPath form template from scratch and follow the instructions on activating the Contact Selector control. If it doesn't work, then we can troubleshoot whatever errors you're getting, but your route should not include anything related to what you've listed here.
http://blogs.msdn.com/infopath/archive/2007/02/28/using-the-contact-selector-control.aspx

My only success came through this process through SharePoint:

- Created a new Document Library named: ContactSelectorTest

- Enabled the option: Allow management of content types

- Added a new column to this library named: Responsibilities and set the data type to Person or Group

- Clicked the Document Content Type and selected Document Information Panel settings

- Clicked Create a new custom template

- Reviewed the data source

p:properties

:documentManagement

    ns2:Responsibilities

       UserInfo

          DisplayName

          AccountId

          AccountType

This is the only way I have got the ContactSelector to link up with the Active Directory. The problem is that everything except for the ns2:Responsibilities field is locked, so I cant add any controls to the form properly (i.e. they are on the form but you cannot edit anything about them nor are they listed in the Data Source). I tried setting editable's value to full in manifest.xsf but that did nothing.

I just want the Contact Selector, which accesses the Active Directory, to sit within a form.

Any help is appreciated.
