2 Forms with the same name

Hi all,

Kindly assist me as I've a problem with my form. The form is a workflow one which has many steps before submitted to an email address. When I submit the form 2 times to email it's received in 2 separate mails although it's the same form with the same name. How can I resolve this problems to have all data in the same form instead of receiving it in 2 separate mails.
