You can pass information to the InfoPath feedback form via another web part. If you click modify page the web part dropdown will give you an option to connect to other web parts. This, however, requires some sort of button in an other web part that sends information. You could have a button called "Feedback Form" and that button could be in a content web part that communicates with your form. That "button web part" could query the page information using javascript. I'm not sure of the incantation, but you could Google for "get browser title jquery script" or something. You'll have to add this code to the button and then connect the web part to the InfoPath form.

Anyway, that's an idea. Let us know if you need more help.

Capturing an unique page identifier (Title, URL, etc) and auto filling a hidden field in the form

I created a feedback form to be at the bottom of a help solution page.

It is simple. It asks "Was this helpful? (yes/no)" and a "Comments" field.

I pull this infopath form in as a WebPart at the bottom of each solution page.

What I am trying to figure out is how I can pull from the page that has the form inserted into it, such as the page Title or maybe URL, capture that info and insert it into a hidden field in the form, so when it is submitted, I have a clue from which page the form was filled out and completed.

I am open to any ideas.

Thanks for any input or help.
