Qdabra&#39;s DBXL tool might be of help - it has a QueryDB web service that allows you to query data from SQL; you can even build complex filters if needed. It also has a submit web service that will submit your data back to SQL. I&#39;d like all questions to appear on the form and users would then select &quot;Yes/No&quot; from the drop-down and submit data to database.</p> <p>&nbsp;Since form is connected to database, I&#39;m not able to pre-populate the questions. Has anyone designed a similar form?</p> <p>&nbsp;Appreciate any help.</p>
