Change the location of the Infopath form

Users enter the name and zip of the student and it launches a Infopath form and the code behind this is as following:

however I would like to launch the form which is in the following location after the infopath form is published in the form library and the location is

http://sitecollection/SL/_layouts/FormServer.aspx?XsnLocation=http://sitecollection/SL/Parent%20Travel%20Permission/Forms/template.xsn&SaveLocation=http%3A%2F%2Fsc%2Esc%2Esc%2FSL%2FParent%20Travel%20Permission&DefaultItemOpen=1

when the form is opened using the publishing Wizard the address is as following:

How do I write the above string href in the following location?
