I am trying to get the form to save as the info in two fields within the form. Here is the formula I am using:

COMPANY_NAME + DATE

When I verify the formula it says its fine...but when I fill out the form and submit it saves it on sharepoint as "NaN". I'm trying to get is to save as something like "Customer10/11/05".

Any ideas why it keeps doing that?

Thanks!
