I faced the same problem and I found two solutions
1- Rename the Form Template and republish it.
2- Clear the InfoPath cache.

Thanks,

1. Original Form Template ID: urn:schemas-microsoft-com-office:infopath:Knowledge-Check:-myXSD-2009.07.14T14-44-42

2. Changed Form Template ID: urn:schemas-microsoft-com:office:infopath:KnowledgeCheck-11022009:-myXSD-2009-07-14T14-44-42

I also tried starting with a fresh SharePoint Form Library. I publish the template as shown in 2 above. When I open, it gives me the template conflict message, showing 1 above. I reopen the template that's from my computer, and it's the way I want it (2).

So for some reason, it changes the name in the properties back to the original when I publish it to SharePoint - maybe based on the ID? Grrrrr...

Re: Template Conflict - Trying to Re-Use a Template...

In the properties, give your form a new name prior to resetting the ID.
