ArgumentException, Exception Message: Item has already been added.

I have at form that pulls data from a SQL DB that works fine as an application-based form, but I cannot get it to pull the data as a Web form (published directly or via CA). I created the UDCX file locally and our server guys copied it up centrally using CA, but the form errors out and never displays.

I created a local version of file.udcx, deleted the previous data connection that was converted to udcx and that was causing the cross-domain issue, re-established the rules in the form using the new local data connection document library file, and publish directy to a document library.

So I published to a brand new document library:

Form ID: urn:schemas-microsoft-com:office:infopath:Server-Monitoring-2:-myXSD-2012-06-14T13-10-40, Type: NullReferenceException, Exception Message: Object reference not set to an instance of an object.)

Any ideas?
