Open form from current form and Pass values

I Have a repeating table(Columns: Name, Designation, Department) which is populated with records from sharepoint list. My requirement is to open a Add details form when a user clicks on the Name column and pass the designation and department values to the Add Detail form. Is it possible to do if yes then how can I achieve this?
