Where are you based?

2. Are you leaving or joining?

Then you can design both checklist on one form and only show the questions/checks that are required based on the 2 qualifying questions (this would make it easier for you to maintain too).

The form in its entirety gets saved into a SharePoint form library. You can publish and show specific data to the library and create views so that 'management' to see and monitor etc.

Prompting on SharePoint to open specific forms

I need to make a form for an onboarding / outgoing checklist for personnel at 10 different stations. I want the user to select which station he/she is assigned, then select whether they're joining or leaving the station. The answer to these prompts will trigger SharePoint to open the correct checklist form. For example, if the user selects the Altoona station, then selects I'm leaving the station, this would open the Altoona outgoing checklist form. I have no idea where to start. Is this a library or list function?
