I would like it so that when a person goes to the SharePoint site they can click New, and then New Item. Once they click that I would like this form I created to show up so the user can fill it out and click the submit button.

I’m having a few problems:

1.	I don’t know how to add the form to the “New” list.
2.	I have submitted a test form but it submits an InfoPath version of the form. I need something everyone can view.
3.	I need people to be able to fill in the form without using InfoPath.

Can someone please give me some direction?
