Merge form issues and 2 simple questions

Hello everyone,
I am currently working on making an easy way for several departments to submit their monthly reports, then for those reports to be merged into one full group report. I have am using Sharepoint, with the form as the list template. When I go to merge my 2 test forms though i get the following error:
The document(s) could not be merged.
The required application may not be installed properly, or the template for this document cannot be opened.
Please try the following:
1. Check the General Settings for this document library for the name of the template, and install the application necessary for opening the template. If the application was set to install on first use, run the application and then try creating a new document again.

2. If you have permission to modify this document library, go to General Settings for the library and configure a new template.

My template loads as an XSN file rather than XML for some reason, but other then that, the directions the error window provides are both vague and ambiguous. How can I correct this problem?

Easy Questions:

1. Why is all the text both in the design and when filling out the form in stuck in capitals for some of the sections and headings?
2. The first 5 to 10 characters of a rich text field are cut off when it is being filled out, how do I eliminate this issue? Again this only happens with some of the sections with no rhyme or reason to it.
