I was guessing that since you'd said this was "choice" column you only wanted them to be able to select LOB or A. Unfortunately, I don't think there is a way to allow you to use an option button with a choice field that allows multiple selections. At this point, you would need two fields - Question1LOB & Question1A - for each question.

Re: Check Boxes

last thing.. I would not use checkboxes for this, but would use radio buttons instead in the form. In the form, in the main data source, select the field for question 1. Click the dropdown and you should have the option to insert option buttons. How about this ex.

1. Today you used a cashiers check. [Check Box] LOB [Check box] A
2. Do customers stay over 10 mins? [Check Box] LOB [Check box] A

Re: Check Boxes

May I see your form?

Re: Check Boxes

I've tried to bind 'Check Boxes' which is the option I want (Check Boxes).

The only thing with that is.. you get two new separate variables. We will view this in the 'Form' via via SharePoint "Edit (link to edit item)".

So yeah.. Using a SharePoint Custom list to create the variable with 2 choices (right now).. then a QC will use the form to rate the record from SharePoint.

Re: Check Boxes

Help me out a little :) Where are you viewing this that you want it side by side?
