Using their Database Accelerator and InfoPath, Qdabra developed a point of care and transcription based electronic medical note application. The nursing staff and physicians enter medications, allergies, family history, problems etc, while interacting with the patient. The physician then dictates the recent history, impression and plan. Since everything is online they can review and dictate at any clinic workstation, or from home if the prefer to spend time with their family and then complete things later in the evening. At the next visit the patients are given a printout containing the information from the previous visit to review as well as a review of systems worksheet. They never have to fill out a blank medical history and medication form again. The transcriptionists open the form once the physician has dictated their portion and complete the note. Not only is there less for the transcriptionists to type, but since they have a list of all new forms it functions as a transcription management system as well. This has streamlined our transcription completion and review process saving physicians valuable time while also creating much more complete notes. Their solution has been up and running for over 2 years without any downtime. We have more than 30,000 forms and 50 staff an physicians using it.
